Team check in for the Glory Cup will take place on Wednesday, July 20th from 5-7pm.
Please bring the following to tournament check-in:
- 5 copies of Affinity photo rosters - Scroll down for instructions on how to create a photo roster.
- Laminated player cards with official team roster may be accepted in place of UYSA Affinity rosters.
- Guest player authorization forms must be provided if you have loaned players from OUTSIDE your club. Club passed players do not need these forms. You can download the proper form here.
UYSA Player Participation Forms/Association approved medical release forms must be easily accessible to your team admin, should a player need medical attention during the event. These are NOT required at team check-in.
Academy teams may check in at headquarters one hour before their first game. Academy rosters can be manually created but must include; Team name; including the team age group, coach's name and contact info, players names and DOB for each player. Academy team rosters are limited to 10 players per team.
All tournament fees must be paid in full by July 15, 2022.
Teams not properly checked-in will not be eligible for tournament play and will forfeit all tournament fees paid.
